Data, purposes and consent
In this version, assessments are calculated in your browser. The questionnaire does not collect contact details or send answers by email. Answers can include health needs: do not enter diagnoses, medicine names or identifying details. If you contact us by email, we use your message to respond to your request. Your selected location and household dependencies are used locally to tailor your plan. We do not request your address or GPS.
Retention and service providers
Drafts and progress you choose to save remain in your browser’s local storage, under your control. Hosting is provided through OpenAI Sites and may process technical access data needed to deliver and secure the service. External retailers apply their own policies when you open a link. This policy will be updated before email services are enabled. Your browser may query GDACS and IPMA public sources for recent information. These connections expose normal web connection data, such as your IP address; questionnaire answers are not sent. Country filtering happens in your browser.
Your rights
You can request information, access, correction or deletion using the contact above. Clear your draft in the questionnaire and remove local data in your browser settings. You may lodge a complaint with the Portuguese data protection authority, CNPD.
Without email delivery, result data stays in this page’s memory. The functional draft is stored in your browser during the questionnaire and cleared when you choose “View on screen”. You can explicitly choose to save checklist progress on your device. Language preference is also stored locally.
